How to Pick Your Surgeon
Reversal of vasectomy is technically possible for most men after vasectomy. Reconnection of the two ends of the vas deferens (the vas deferens is divided during vasectomy) must be accomplished with the aid of an operating microscope in the operating room under anesthesia. If sperm are still present in the vas deferens, then the operation has better than 95% chance of returning sperm to the ejaculate in my experience. If a long period time (over 10 years) has passed since your vasectomy, then the chances of sperm being present in the vas is lower, and a more complicated procedure may be needed to "reconnect" your system. This may require connecting the vas deferens to an area closer to the testis called the epididymis. Regardless of the findings at the time of surgery, all possible efforts will be made to successfully reverse your vasectomy.
Money Back Guarantee?
Let us examine whether money back guarantee really provides any reassurance of you meeting your goal of having a pregnancy. Success after a vasectomy reversal can be measured several ways. What is important for you is whether the operation will allow you to have a child. The "technical success" rate and the overall pregnancy success rate are different. Through my experience with working with many infertile couples I know that the final quality of the semen after a reversal is very important. I strive to complete the reversal as perfectly as possible to restore ejaculate to its original quality, usually around 60-90 million sperm. This mission started with my microsurgical fellowship training at Cornell Medical Center in New York and is supported by the dedication of my surgical team and surgical center, who understand the challenge and difficulty of microsurgery.
"Technically successful" reversals may result in sperm counts of 1-20 million sperm. These individuals will have much lower if no successful pregnancy. Money back guarantees on a vasectomy reversal are usually void if they are "technically successful." You may end up with 5 million sperm after a reversal, no pregnancy and no money back in your pocket! The operative technique and surgeon greatly determine your final sperm count by controlling whether the vas connections will be open 100% or less after the surgery. Choose your surgeon carefully!
The before surgery and after surgery care is very important! This is where my male infertility training is very important. I closely evaluate before surgery every vas reversal patient for any condition that may decrease the success of an otherwise excellent microsurgical reconstruction. It is important to identify any condition that is limiting the sperm producing capacity of the testicles. A man's sperm quality may decrease since his last child for a variety of different reasons. He may have harmful lifestyle habits, ongoing medical problems, hormonal problems, ejaculatory problems, prostate problems, or male genital problems that may contribute to a low quality semen sample. Many of these conditions are easily reversible and if addressed before surgery can significantly decrease the time to a successful pregnancy.
After a vasectomy, the body can mount a reaction against the sperm. This process results in antisperm antibodies which cause poor sperm motility (swimming) and poor sperm function. It is important that this be diagnosed and addressed. This is one of the reasons a close follow up after surgery is important. I will diagnose this condition and provide a low cost means to overcome this problem.
Sperm freezing during surgery is occasionally necessary and is always available for all patients. Some patients feel secure with a back-up sample. I only truly recommend it if I feel by exam that the vas reversal may be very difficult.
"The microsurgical skills and judgement are critical for the success of a vas reversal. The before surgery and after surgery care assures that a man's fertility is maximally enhanced."
Because the chance of a successful pregnancy after a vasectomy reversal is not assured, you may strongly benefit from working with a surgeon who is not only an expert microsurgeon but who can provide many techniques to improve your overall fertility. Your overall fertility may be somewhat decreased since the time of your vasectomy.
The vasectomy reversal will be performed on both sides (for each testis that is present) and usually takes 3-5 hours of operating time. You will be asked to come to the surgery center the morning of surgery, and most men are able to go home the same day. Occasionally, it may be necessary to place a small rubber drain into the scrotum during surgery that is painlessly removed the next day. Dr. Williams will meet with you in the office prior to surgery to review details about the operation and recovery.
Dr. Williams will discuss your personal success rate with you after your examination in his office.
