What to Bring to Your Appointment
- A comprehensive infertility and sexual history form for the man and his female partner (After making your appointment, please call 832.358.8600 to have the form sent to you.)
- Your insurance card and knowledge of any infertility benefits covered under your insurance plan
- Copies of reports from any previous male infertility evaluations, including a complete report of previous semen analyses

Payment and Insurance
Payment for physician services
Payment for physician services is expected at the time of your initial visit. Some physician services are covered by insurance. Please call our outpatient and pre-certification personnel for assistance at (832) 358-8600. Your insurance carrier can provide further information on infertility benefits.
Payment for laboratory services
Payment for laboratory services is collected at the time laboratory tests are ordered. A receipt will be provided if you choose to file a claim with your insurance carrier. You may or may not be reimbursed by your insurance company for our services. Please check with your insurance carrier about available infertility benefits.
